Insurance


We accept most insurance plans. If you have questions about your coverage at our facility, please call your insurance company.


Co-Payments / Co-insurance


Co-payments and co-insurance amounts are determined by your insurance company and based on specific plan benefits. Please check with your insurance company to see what your co-payments are prior to appointment.

 You can schedule an appointment by clicking "new patient" under the "patient Center"  tab or you can call our office at (940)-383-3444. 


Our regular hours are Monday- Friday (8am-5pm) . We offer a 24/7 On call service for questions and after hours care by a trained professional- Please call 


We accept most major insurance plans. If you’re unsure, feel free to call us, and we’ll help you verify your coverage. 


Please bring your photo ID, insurance card, a list of medications you’re currently taking, and any medical records or test results you think may be helpful. 


Yes! We’re currently accepting new patients. Call us or visit our New Patients page for more info on how to get started. 


Yes, we provide routine vaccinations, annual wellness exams, sports physicals, and school physicals.


 We kindly ask for at least 24 hours’ notice. You can cancel or reschedule by calling our office


Yes! Please call our office or request a refill through the patient portal. Refills may take up to 48 hours to process. 


Yes, our trained phlebotomist will retrieve samples with expert precision and ease.
